Can someone with more tactical nuance and understanding of the game explain to me why we look so empty in midfield and teams bypass us? 

Also why do we always look like we're defending even with the ball? We just get pushed back passing side to side between the defenders. There isn't a single pass through the middle to draw players to the ball.
